The number of patients with tooth wear has increased dramatically during the last 10-15 years. When the wear is severe and pathologic, a rehabilitation of the worn dentition is needed. In younger patients a dynamic, minimally invasive, adhesive treatment concept is preferred nl. a localized or complete build-up of occlusion with direct composite restorations. This restorative treatment has to be planned using study models, intra-oral and extra-oral pictures and a diagnostic wax-up. In this course, the different aspects of localized/complete rehabilitation of tooth wear of direct restorations will be covered. Several methods to build up the teeth in the posterior region (using a transparent silicone key and a conventional composite system, freehand modelling and a conventional composite, and the injection flowable technique) will be discussed and practiced in a hands-on. Finally, the durabiliy of the direct composite restorations placed for the treatment of severe tooth wear will be shown with the help of several cases after 5-10 years of clinical functioning.
